Why do I need to complete the Allianz Waiver of Travel Insurance Coverage form if I’m not purchasing travel insurance?
This form confirms that you’ve been offered travel insurance through Allianz and allows you to either accept or decline coverage. You must indicate your choice by checking or leaving the box unchecked, and then print your name, sign, and date the form. This ensures you understand your options and acknowledge any potential risks of traveling without insurance, such as trip cancellations, medical emergencies, or lost luggage.

Why do I need to complete the Credit Card Authorization form, and what does the Travel Insurance Waiver section mean?
This form authorizes InteleTravel (or your Independent Travel Agent) to process payment for your travel arrangements directly with approved suppliers. It also includes a Travel Insurance Waiver section.
You must choose and sign one of the following options:
Accept and authorize the travel purchases including travel insurance – sign and date to confirm you are purchasing insurance.
Accept and authorize the travel purchases but decline travel insurance – sign and date to confirm that you understand and accept responsibility for any risks of traveling without coverage, including non-refundable tickets, penalties, or change fees.
Final travel documents (tickets, vouchers, etc.) cannot be issued until this form is signed and returned, along with a legible copy of the front and back of your credit card for verification.
